I used to do the intake forms with people in my office during the first few minutes of the first session, but this chore is really better done in advance.
That way we do not waste our time together on paperwork.
If we are starting with therapy over the phone or online, please download, print, and complete the informed consent to telemedicine first.
If possible, also make a copy of your insurance card, front and back, on one sheet.
A black-and-white photocopy is fine or a PDF, but please, no JPEGs, which don't fax well.

Marcia E. Brubeck, JD, MSW, LCSW, is a graduate of the University of Connecticut Schools of Law and Social Work.
In addition to being a member of the Connecticut bar and a licensed clinical social worker, I am a diplomate of clinical forensic counseling, a certified school social worker, and a member of the World Professional Association for Transgender Health (WPATH).
Prior to opening my private practice in 2000, I provided individual, group, and family therapy in inpatient and partial hospital settings and in extended day treatment programs for children.
A terrific self-help manual choc-o-block full of great ideas designed to improve relations between a husband and wife.
What I liked most about Marcia's ideas is that she helped me become aware of the whole picture, not just viewing situations from my own perspective.
With each positive resolution we come to with our husbands, by being proactive and honest, the more we are empowered to keep the positivity going.
